July Summer Visit Day

I understand and acknowledge that use of and participation in recreational facilities at the Hadlock Student Center and University Recreation programs is entirely voluntary and involves serious risk including but not limited to risk of property damage, bodily injury, permanent disability, paralysis and death. These risks may result from the use of the Hadlock Student Center, use of other recreational facilities, participation in University Recreation programs, the acts of others, or the unavailability of emergency medical care or immediate staff response.

I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my use of and participation in any recreational facilities and programs. My participation in this activity is purely voluntary, no one is forcing me to participate and I elect to participate in spite of and with full knowledge of the inherent risks.

In consideration of the services of George Fox University (GFU), I release GFU and all of its subsidiaries, affiliates, agents, employees, directors, officers, shareholders and attorneys; and any and all other persons or entities, known or unknown, who are or may be claimed to be liable; and all of their respective predecessors, successors, heirs, attorneys and assigns; from any and all claims and causes of action, known or unknown, arising from or relating to the use of the Hadlock Student Center and participation in University Recreation programs. I certify that I am fully capable of participation in all activities offered by University Recreation that I choose to participate in. Therefore, I assume and accept full responsibility for myself for bodily injury, death or loss of personal property and expenses as a result of those inherent risks and dangers not specifically identified and, as a result of my negligence in participation in this activity.
